In our practice we have some parents who want to donate for their kids but for some reasons ( ABO incompatible) they are excluded
If we had this type of kidney paired donation it would be a great management

In Brazil we had the first KPD at the end of last year. Still embryonic, requiring more robust logistics and extremely necessary in a patient where more than 90% is from deceased donors and with an extremely mixed genetic structure.

Dual kidney donation, or pair exchange, is an approach to living donor kidney transplantation in which patients with incompatible donors exchange kidneys for a compatible kidney.
KPD is used in cases where the potential donor is incompatible.
I hope to God that it will be implemented in our country.

The video of the hand assisted laparoscopic nephrectomy is very nice; it refreshed my knowledge (my knowledge here is indeed very meager, I am not a surgeon) in anatomy.
In our country(Sudan),unfortunately, no surgeon is doing laparoscopic nephrectomy; although some of them are certified by some recognized international bodies.

we have some experience with PKD it is far better than desensitization and risking for more AMR, heavy immunosuppression and therefore more infections. But the concern in some countries, like in our middle east, beside the logistics is the fear that one donor may escape once his relative has been transplanted. Any such concern in other regions, or such experience?

Thanks, Montasir There is always a concern that one of the pairs may change mind. Therefore; all pairs are done at the same time.
